Child Passport

PARENTAL CONSENT
Every child under the age of 16 requires consent from both parents listed on the child’s birth certificate or legal guardians in order to have a passport issued. Children ages 16 and 17 only required consent from one parent.
Consent can be obtained by both parents going to the Acceptance Agent with the child. If one parent is unable to go to the Acceptance Agent they are able to submit a notarized consent. If the absent parent is not present in the child’s life, court papers may be submitted showing the applying parent has sole custody.

CURRENT US PASSPORT

PARENTAL ID & CONSENT
Requirements
Consent can be obtained by both parents going to the Acceptance Agent with the child. If one parent is unable to go to the Acceptance Agent they are able to submit a notarized consent. If the absent parent is not present in the child’s life, court papers may be submitted showing the applying parent has sole custody.
You must submit documentation that lists the parent(s) or legal guardian(s) of the child applying for a passport.
The following may be used to show the parental relationship:
- U.S. birth certificate (also evidence of U.S. citizenship)
- Consular Report of Birth Abroad or Certification of Birth (also evidence of U.S. citizenship)
- Foreign birth certificate
- Adoption decree
- Divorce/Custody decree
The parents/guardians must present one of the following identification documents (IDs) to the acceptance agent:
- In-state, fully-valid driver’s license
- Valid or expired, undamaged, U.S. passport

When applying to have the name on your passport change, you must submit legal proof of the name change. You will be required to send in the original document; photocopies will not be accepted.
